  1. It can be down to a number of factors really.

Tyres too cold, bad track surface, bad tyre pressures / suspension setting, too much power too soon, pushing the front too hard.

If you are god like ( motogp class) you can save a fall. Like Colin Edwards here : http://uk.youtube.com/watch?v=3TfWV9bw_z... or even Nicky Hayden here : http://uk.youtube.com/watch?v=DLArUT30F0...

Even modified road bikes in Superbike class can bee ridden to amazing lean angles as shown here by US rider, Ben Spies : http://www.superbikeplanet.com/image/200... .

  2. when taking corners on a road racing some riders go into the corners so fast that when braking to go round the corner the weight of the bike is forced to the front of the bike and the rear wheel loses tracktion ....in saying that when a rider exits the corner and gives to much throttle the bike could lose traction and you would see the sliding affect

  3. when they are going round corners the wheel can sometimes slip due to they timing of the breaking/acceleration and lack of traction.

you will sometimes here riders on interviews saying that they just 'lost the rear wheel'.

  4. There is a lack of traction (friction) between the spinning wheel/tyre and the dirt track surface.

With the bike leaning over, gravity pulls on the side of the bike to pull it down.

  5. Because they lean too far perhaps!!
