  1. Go to your local pet store to get a kit to check the PH balance in the water.  Is this a new tank?  Have you moved and are using new water? Is the tank in direct sunlight?  Do you use an algea killer and or chlorine controller?  You also might check that your filter is working correctly.  Wait, are we talking salt or fresh water tank?  Check all of those, then go from there.

  2. u might be putting a little bit too much fish food, if youre not, then you should get ph balance and you should check your filter.

  3. If your tank looks like a milky white, chances are you have a bacterial bloom.  This is usually accompanied by new tank syndrome and means your tank is trying to cycle.  Over the next few days to a week, the water should gradually clear on its own.  If your tank is cycling, there is a chance your fish will die, as your ammonia and nitrite levels will skyrocket.

If the tank is a milky white/green, you have an algal bloom.  Algal blooms can be caused by many things including too much direct sunlight and high phosphates.  Uneaten food is a great fertilizer for algae.  I'd suggest cutting down the amount of food your feed your fish and carrying out a partial water change if this is the case.  There are also products you can buy in the pet store that fight algal blooms.
