Why is when people put do not restatiate on their medical wills or something they asked for it?

It is all about choice. They want to have the choice to live or not. Why, only they know,  I do not think it means they want to die, I just think they do not want to be resuscitated and not be how they were, the what if I survive and am paralyzed, or not the way I was to begin with.
