Without detailed info the possibilities are many.

You created the ava but did not save it.

You saved it but did not okay it for use on yahoo page.

You okayed it but did not specify the name you want to use it with.

You specified it but did not save/activate the specification.

You saved/activated it but did not apply it to the page with a checkmark next to the name that's associated with the page...

Did I leave anything out...?
