Your weight distribution is the main item here (as far as physics goes). On a level surface (smooth road) the weight distribution is always the same and very predictable, so you can run with less difficulties, you do not have to keep compensating the load distribution.

You are constantly twisting the ankle,on uneven surface, if you sprain it you cannot even work or have difficulty walking even (this is the physiological part of the equation, it is like which one came first egg or the chicken).
