They will pay for a portion of a gastric bypass ( NOT LAP-BAND) if you meet the following criteria...

If you have been under a medically supervised weight loss program for 6 or more months with a minimum of 12 physician visits and monitoring... without success.

You are 100 pounds or more overweight ( morbidly obese) and have a Body Mass Index (BMI) of 40 or more...

I was put on high dosage steroids, and gained more than 100 pounds which I have only been able to take off 15 pounds of in three years time. MY BMI (seeing how I was normal weight before gaining the weight) right at this minute is 34 even with the more than 100 pound weight gain...
