Work Insurance...Need Help...simple question

The sheet is titled "Evidence of Insurability"...and there two checkboxes but I don't know which one I should check off or if I even need to check it. The first one is: "Insurance Amount Over the Non-

Evidence Limit"

The second is: "Optional Life"

I just need it for myself so which one do I tick off?

What kind of insurance is this?  Life, health, credit, homeowners?

Don't check either of them.  

You'll need to show that to your next health insurance company.
