Can that form be used by anyone in any location? Because I live in Parkesburg, PA and was wondering if I could download that PDF instead of having to go to my school to recieve the form.

Thanks in Advnace.

  1. Forms used throughout the state should be the same, but verify that.  Check with your state's Dept of Labor -- their offices should be in or near the state's unemployment offices.  You need your birth certificate to prove you are old enough, photo ID (perhaps your school ID or state ID or learner's permit/drivers license), and your parents need ID to prove they're your parents to give their permission for you to work.

If you and your parents are not US-born citizens, you need to prove naturalization or legal permanent residence.  Those on visas (as a child, you would be on a dependent's visa and not authorized to work) or not legally present are prohibited from working by federal law and are ineligible for working papers.
