if one of your client were moving away from there family home in to a supported living service what kind of support would you offer them??/

  1. A social worker at a hospital can tell you what services are available and there are so many.

    This is an untapped resource usually.


  2. Would look at their abilities/needs.

    Try to find somewhere in the area, where family can visit regularly.

    Ask them their interests, if they want to find work or a day centre placement.

    Would try to place with adults of a similar age group and ability range.

    Would offer the chance to go look round the house and meet the other residents, possible stay fo r tea.

    Would offer a weekend stay before they decide to move in permanently

    Would discuss with client/family what is important to them.

    Would look if the client follows a special diet, and if the home can cater for this.

    Would check home has  adequate staffing so client can be supported out regularly

    Would hold regular review meetings to see if client is settling and to ask them if they are still happy living there

    Woulod make sure client knows who to contact if anything is worrying them, or if they are unhappy

    List is not exhaustive, and I know that not all of these steps are properly followed all the time, but it makes a good start
