Worst experience at a sit-down restaurant?

What is yours? Here's mine:

I went with three of my friends to Applebee's. When the waitress came to take our order, she just stood there with her pad and pen, waiting. No smile, no greeting, nothing. We ordered our food and drinks, and when we got them, she practically threw them on the table and stormed off. Not once did she come back to ask how our food was or if we wanted refills on our drinks. (none of us knew this girl in case you are wondering.) We left her a 50 cent tip and reported her to the restaurant manager, and I haven't seen her there since.

Well i was at Ruby Tuesday's and there was a dirty knife on my seat and i sat on it! luckily it didnt leave a scar, and the food was cold when it came but thats not as bad as the knife..........

Sorry , I thought you said, "restaurant". To me Applebee's, etc is a diner with a fancy vernier.

However, my experience was to be seated at the wait door of the kitchen, in a high volume Italian restaurant, on Valentines Day. The door was slamming and banging the entire time. Terrible dining experience.
