  1. If you have a PPO plan, you need to find out what your travel network is.  (HMOs have different rules on out of state non-emergency care, and you'd have to check directly with your insurer on that.)

    But for a PPO...check your insurance card - there should be some logos on the back which indicate what networks you'd use if you were out-of-state and needed care.  (There are several large national networks which exist for this reason...some common examples are Beech Street, First Health, Multiplan, PHCS, etc.)  Once you find out what PPO network you'd access when out of state, then you can determine whether the Mayo Clinic or Duke Medical are part of their network.

    I know that the Mayo Clinic is contracted with many of the large national PPO groups that I listed.  (I used to work for a large regional health insurer...we had members who occasionally sought care at the Mayo Clinic.)  Again, once you know what specific out of state network you'd be accessing, then you can find out whether Mayo or Duke are part of that.

    (In addition to checking the logo on the back of your card, you can also call your insurer directly and ask them what network would be considered "in-network" for care in Minnesota or North Carolina.)
