Write it like a cover letter for a resume, since they will already have your transcripts in hand.  Introduce yourself, talk about your educational history, your future career aspirations, how your post-secondary educational goals are set, and why you would be a uniquely excellent candidate for their school.  Be sure to include any apt adjectives such as:  tenacious, determined, hardworking, goal-oriented, invested, committed, dependable, trustworthy, etc.
