Writing a formal letter of withdrawal from a magnet school ?

i need some help writing a formal letter to withdraw myself from it. is it fine the way it is? does it even make sense? The school just told me that i need something written on paper telling and confirming that i am not attending the school. Here it is so far and also, its suppose to be my mother writing it but i am instead -.-;; :

This is to inform you that my daughter, ---------, is withdrawing from the Magnet Program of -------high school. She is unable to attend this school because we moved out of state. Accordingly, this correspondence is in compliance with your requirement of a formal withdrawal. Should you have further requirements, please advise.
