proof for Chelsea disabled ticket

I would like to know that what document I should provide with membership form to get Disabled membership of Chelsea FC.

 Receipt of the middle or higher rate of the Disability Living Allowance (mobility or care component), OR Receipt of either the Severe Disablement Allowance or Attendance Allowance, OR A personal letter from the hospital or your GP confirming that you are a disabled person with a recongnised impairment that requires extra help, OR A personal letter from the local authority social services department that the person is in receipt of support services.
