  1. It has a slightly mischievous connotation, as though hoping to hear that the person who is asked it has been hatching some little plot or engaged in some activity that is a little more interesting than the standard routine. Up to no good = planning some mischief.

  2. "What have you been up to today?" is a question that asks what you have done all day, like work or leisure activities, in the past tense.

    Example:

Mary is walking down the street and sees Joe.  She hugs him and asks him, "What have you been up to today?"  He responds, "I was at work until 5:00pm, and then I went to the store to pick up some milk."

    PS

It doesn't mean anything sexual, which is what some people have already suggested.  Just ignore them.  And "What have you been up to today?" is not really conversation filler.  It's an actual question that gets actual responses.  Conversation filler that is similar, though, is "What's up?"  And I think that's what one of the other people who answered your question were thinking of.

  3. if someone asks you this, they are simply trying to find out what you have done throughout the day, or perhaps your plans for later, or whether or not you are busy. It is typical American small talk.

  4. It's usually idle chit-chatter of a bored person. I ask that question to seem interested or to get a conversation started. Like hey how ya doing? Most people respond like this- I'm good how bout yourself? Even if their life is crappy because no one really wants to know how ya doing.etc. It's fluff.

  5. For most Americans if they say this it is usually a conversation starter that means what have you done today. It is meant purely for means of creating a conversation or getting one going.

  6. It means quite literally, "How many erections have you had today?" yes, it may even be said to females...it's an idiom sillies! well, where I come from at least!
